Care Coordinator Job – We Are Recruiting on Behalf of Our Client
Careline Solutions is recruiting on behalf of a valued client for an experienced Care Coordinator to join their domiciliary care team in Kidlington.
This is an excellent opportunity for an organised, compassionate and proactive individual with previous experience in care coordination or scheduling within a domiciliary care setting. The successful candidate will play a key role in coordinating care services, supporting care staff and ensuring clients receive safe, reliable and high-quality person-centred care.
Working closely with the Registered Manager, you will coordinate care visits, manage staff rotas using CareLineLive, maintain excellent communication with clients and care workers, and ensure services are delivered in line with CQC standards.
Key Responsibilities
Care Coordination
- Create, maintain and manage staff rotas using CareLineLive to ensure all care visits are allocated efficiently.
- Match care workers to clients based on skills, experience, preferences and continuity of care.
- Respond promptly to changes in staffing, client requirements and emergency situations.
- Monitor care schedules and make adjustments where required.
- Ensure all care visits are delivered in accordance with individual care plans.
- Participate in the on-call rota on a rotational basis.
Client Support
- Act as a primary point of contact for clients and their families.
- Liaise with healthcare professionals, social workers and other external agencies.
- Assist with setting up new care packages and coordinating service commencement.
- Ensure care plans and client records remain accurate and up to date.
- Handle client enquiries and concerns professionally and efficiently.
Staff Support
- Maintain regular communication with care workers.
- Provide day-to-day operational guidance and support.
- Assist with staff induction, supervision and ongoing development.
- Monitor attendance, punctuality and staff performance.
- Promote staff wellbeing and encourage a positive working environment.
Compliance & Quality
- Ensure care services comply with CQC regulations, company policies and relevant legislation.
- Maintain accurate electronic and paper records.
- Report safeguarding concerns, incidents, accidents and complaints promptly.
- Contribute to continuous quality improvement initiatives.
Administration
- Update CareLineLive and other care management systems accurately.
- Prepare reports and maintain confidential records.
- Monitor care hours and support payroll administration.
- Assist with recruitment administration and onboarding processes.
- Participate in the on-call rota as required.
Person Specification
Essential
- Previous experience as a Care Coordinator, Scheduler or Care Planner within domiciliary care.
- Good understanding of person-centred care.
- Excellent organisational and time management skills.
- Strong communication and interpersonal skills.
- Ability to work under pressure and manage competing priorities.
- Competent in Microsoft Office and electronic care management systems.
- Good knowledge of safeguarding procedures and CQC regulations.
- Full UK Driving Licence and access to your own vehicle.
Desirable
- NVQ Level 3 in Health & Social Care (or equivalent).
- Experience using CareLineLive or similar care planning and rostering software.
- Experience supervising or supporting care staff.
- Knowledge of local health and social care services.
